Question: What system of notation dates back thousands of years and is still being used by some pharmacy practitioners today?

Answer: "Roman numerals" are still used today because of an ancient system of measurement known as the "apothecaries," which requires them. An example would be writing the abbreviation of 6 grains, which is written as gr.vi. The "apothecaries" system is discouraged in contemporary pharmacy practice, but may still appear on prescriptions and compounding formulas.
